Bring the following three items:
- Most recent power bill (this not necessary for assistance with oil, propane, or wood expenses)
- Social Security cards for everyone living in the home. If you do not have cards for everyone in the home, you can order new copies from the Social Security office.
- Proof of Income for the last 90 days (3 months) for all household members. See notes below:
- If anyone in your household over 18 years of age has zero income, he or she must come to our offices to fill out and sign a form certifying zero income. Alternatively, this form can be found here on this website. Print this form, have it filled out and signed, and bring it to your appointment.
- We cannot accept bank statements as proof of income. An annual award letter serves as proof of income for SSI or SSDI that is deposited directly in a bank account. Statements of Social Security awards can be obtained at the Social Security office.
- If you are receiving TANF, a copy of your benefit information can be obtained from the DHS office.
- If you are receiving child support by direct deposit, we cannot accept bank statements as proof of income. If you have no proof of this income, , ask for information about how to obtain a copy of your payment record online from Support Enforcement.
- If you are receiving unemployment that is direct deposited, we cannot accept bank statements as proof of income. At the time you make your appointment, ask for information about how to obtain a copy of your payment record online.
