1. General Observations: How is the child? Who Lives in the house? Living Conditions, etc.
2. Positive news to share.
3. Concerns, what coudl be better (from CASA, child, and/or foster parents)?
4. Are the children seeing a doctor, dentist, etc? Last medical Check-up/exam?
5. Are the children in therapy or counseling? Name of therapist/counselor?
6. Are the children having regular visits with parents, family, and siblings? Any concerns or barriers?
7. Any other information important to the child and situation.
